Job summary

Healthscope Independence Services (HIS) is seeking a passionate Disability Support Worker to provide high-quality care to clients under the NDIS in Mount Macedon, VIC. You will assist with daily living activities, personal care and community engagement, helping clients achieve independence.

The role requires Certificate III/IV in Disability or related qualification and at least 1 year experience in disability or healthcare services.

Qualifications

• Minimum 1 year of experience in disability or healthcare services.
• Strong communication, written, interpersonal, and organisational skills.
• Flexibility to work across a range of shift types.

Responsibilities

• Provide personal care support in line with individual support plans.
• Assist participants with daily living activities including meal prep and feeding.
• Assist with household tasks such as cleaning and laundry.
• Accompany and support clients in community activities, appointments and recreation.
• Ensure documentation (daily notes, progress reports) is thorough and up to date.

About theRole

As a Disability Support Worker, you will be responsible for providing high‑quality care and assistance to our clients, helping them achieve their goals and live independently. You will work closely with our clients, supporting them with daily living activities, personal care, and community engagement.

A typical day may include:

Provide personal care support in line with individual support plans

Assist participants with daily living activities in meal preparation and feeding.

Assist with household tasks such as cleaning, laundry, etc

Accompanying and supporting clients in community‑based activities, such as social outings, appointments, and recreational pursuits

Ensure all required documentation (including daily notes, communication and progress reports) is thorough and up to date.

Experience with behaviours of concern

Adhere to all organisational policies, including medication administration, WHS, client rights, privacy, and confidentiality.

To be considered for this role you will have:

Certificate III / IV in Disability, Community Services, or related qualification.

Minimum of 1 year of experience in disability or healthcare services is required.

Strong communication, written, interpersonal, and organisational skills.

Flexibility to work across a range of shift types.
