Job summary

New Age Support Services is seeking a Disability Support Worker to provide personalised 1:1 community and in-home support for participants with complex needs in Ballarat. The role focuses on mental health, AOD-related needs and behaviours of concern.

You will apply positive behaviour support, help with independence and daily living, and maintain clear progress notes and risk assessments. A calm, respectful and professional approach is essential.

Qualifications

• NDIS Worker Screening Check is required.
• Working with Children Check where required.
• Current First Aid and CPR certification is required.
• Relevant qualifications and/or demonstrated experience in disability, mental health, community services or AOD support.

Responsibilities

• Providing 1:1 community and in-home support.
• Supporting participants experiencing mental health challenges.
• Supporting participants with AOD-related needs.
• Working with participants who may display behaviours of concern.
• Using positive behaviour support and de-escalation strategies.
• Supporting emotional regulation and maintaining a calm environment.
• Community access, appointments and social participation.
• Supporting participants to develop independence and everyday living skills.
• Following behaviour support plans, risk assessments and participant support plans.
• Completing accurate progress notes and incident documentation.
