Job summary

Lifestyle Solutions is seeking a dedicated Disability Support Worker in Bathurst, NSW. You will assist customers to live more independently at home and in the community, including personal care and medication management, with a strong focus on respectful, person-centred support.

Responsibilities include mentoring, coaching daily living skills, documenting notes, and engaging customers in community activities, with shifts that cover mornings, afternoons, evenings, sleepovers and weekends.

Qualifications

• Must have driver's licence and full comprehensive insurance.
• Diabetes knowledge or training is ideal.
• Peg feeding knowledge is ideal.
• Medication administration and adherence to individual support plans.

Responsibilities

• Mentoring and skill development – guiding customers to explore interests.
• Coaching independent living skills such as cooking and domestic tasks.
• Administering medication and adherence to individual support plans.
• Completing clear, concise and factual handover notes and incident reports.
• Providing personal care such as toileting, showering, feeding and dressing as required.
• Helping customers engage with communities through Community Participation Programs.
• Community access will be required at times.

Lifestyle Solutions provide innovative and flexible person-centered support for people living with a disability. Support Worker to assist a customer to become more independent, in their own home and in the community.

As a Support Worker your responsibilities will include:

• Mentoring and skill development – guiding customers to explore and develop their personal interests/hobbies
• Coaching independent living skills such as cooking, domestic and social skills
• Administering medication and adherence to individual support plans
• Completing clear, concise and factual handover notes and incident reports
• Providing personal care such as toileting, showering, feeding and dressing when required
• Helping customers engage with their local communities and culture through access to Community Participation Programs and recreational activities
• Personal care is a must, and must be comfortable and knowledgeable in doing so.
• Diabetes knowledge or training is ideal
• Peg feeding knowledge is ideal
• Community Access will also be a requirement at certain times.

• Be willing to work a variety of shifts which includes mornings, afternoons, evenings, awake shifts, sleepovers and weekends
• Versatile communication skills including the ability to interact with both verbal and non-verbal customers
• Current driver's license (mandatory requirement)
• Current NDIS Worker Screening Clearance (or be willing to obtain)
• Current First Aid Certificate, HLTAID003
• Formal qualifications in Disability, Community Services, or similar, would be advantageous
