Job summary

Lifestyle Solutions is seeking a Disability Support Worker to assist clients in Day Program and Individual Supports across Minchinbury and Glenmore Park NSW. The role involves personal care, community participation, and helping customers become more independent at home and in the community.

You will mentor clients, coach daily living skills, administer medications per plans, document clearly, and support a compassionate, reliable team.

Qualifications

• Experience supporting people with disabilities in home or community settings.
• Ability to administer medications according to individual plans.
• Strong communication and documentation skills.
• Comfort with personal care tasks when required.
• Current driver's licence and willingness to travel between sites.

Responsibilities

• Mentor and develop clients' personal interests and skills.
• Coach independent living skills such as cooking and domestic tasks.
• Administer medication and adhere to individual support plans.
• Complete clear handover notes and incident reports.
• Provide personal care as needed (toileting, showering, feeding, dressing).
• Support community participation and access to activities.
• Maintain professional, compassionate engagement with clients.

Job description

• Disability Support Worker (Day Program and Individual Supports)
• Minchinbury and Glenmore Park NSW
• Must be happy to do some personal care
• Individual Supports (1 on 1 and 1 on 3 ratio)
• Must have Drivers Licence and Full Comprehensive Insurance.

Lifestyle Solutions provide innovative and flexible person-centered support for people living with a disability. Support Worker to assist a customer to become more independent, in their own home and in the community.

As a Support Worker your responsibilities will include:

• Mentoring and skill development – guiding customers to explore and develop their personal interests/hobbies
• Coaching independent living skills such as cooking, domestic and social skills
• Administering medication and adherence to individual support plans
• Completing clear, concise and factual handover notes and incident reports
• Providing personal care such as toileting, showering, feeding and dressing when required
• Helping customers engage with their local communities and culture through access to Community Participation Programs and recreational activities
• Personal care is a must, and must be comfortable and knowledgeable in doing so.
• Epilepsy management knowledge is essential.
• Community Access will also be a requirement at certain times.
• Strong understanding of mental health conditions (depression, anxiety, schizophrenia, mood disorders).
• Understanding of mental health crisis management.
• Basic computer skills (data entry, record keeping, scanning, printing).

• Be willing to be work a variety of shifts which includes mornings, afternoons, evenings, awake shifts, sleepovers and weekends
• Versatile communication skills including the ability to interact with both verbal and non-verbal customers
• Current driver's license (mandatory requirement)
• Current NDIS Worker Screening Clearance (or be willing to obtain)
• Current First Aid Certificate, HLTAID003
• Formal qualifications in Disability, Community Services, or similar, would be advantageous
• Be compassionate, reliable and punctual
