Job summary

Lifestyle Solutions is looking for a dedicated Disability Support Worker in Goulburn, NSW to support customers in day programs and 1-on-1/1-on-3 environments. The role requires providing personal care and helping clients build independence at home and in the community.

Your responsibilities include mentoring, coaching daily living skills, administering medications per plans, and documenting care through handover notes and reports.

Qualifications

• Experience in disability support settings is preferred.
• Ability to mentor individuals to build independent living skills.
• Knowledge of medication administration and support plans.

Responsibilities

• Mentor customers to explore personal interests and develop skills.
• Coach independent living skills (cooking, domestic, social).
• Administer medications and adhere to individual plans.
• Complete clear handover notes and incident reports.
• Provide personal care as required (toileting, showering, feeding, dressing).
• Facilitate community engagement and participation programs.

• Must have Drivers Licence and Full Comprehensive Insurance.

Lifestyle Solutions provide innovative and flexible person-centered support for people living with a disability. Support Worker to assist a customer to become more independent, in their own home and in the community.

As a Support Worker your responsibilities will include:

• Mentoring and skill development – guiding customers to explore and develop their personal interests/hobbies

• Coaching independent living skills such as cooking, domestic and social skills

• Administering medication and adherence to individual support plans

• Completing clear, concise and factual handover notes and incident reports

• Providing personal care such as toileting, showering, feeding and dressing when required

• Helping customers engage with their local communities and culture through access to Community Participation Programs and recreational activities

• Personal care is a must, and must be comfortable and knowledgeable in doing so.

• Epilepsy management knowledge is essential.

• Community Access will also be a requirement at certain times.
• Strong understanding of mental health conditions (depression, anxiety, schizophrenia, mood disorders).
• Understanding of mental health crisis management.
• Basic computer skills (data entry, record keeping, scanning, printing).

• Be willing to work a variety of shifts which includes mornings, afternoons, evenings, awake shifts, sleepovers and weekends

• Versatile communication skills including the ability to interact with both verbal and non-verbal customers

• Current driver’s license (mandatory requirement)

• Current NDIS Worker Screening Clearance (or be willing to obtain)

• Current First Aid Certificate, HLTAID003

• Formal qualifications in Disability, Community Services, or similar, would be advantageous
• Be compassionate, reliable and punctual
