Job summary

Equity Works Assoc. Inc. on the Sunshine Coast is seeking compassionate Lifestyle Facilitators to support people with disability in home settings, including short-term accommodation and SIL roles.

You will deliver person-centred active support, assist with daily routines and promote independence, while following care plans and maintaining clear documentation. Onboarding, supervision and a strong community focus are provided.

Qualifications

• Experience supporting people with disability (or willingness to learn).
• Values-driven approach grounded in respect, inclusion and human rights.
• Strong communication and relationship-building skills.
• Flexibility to work a range of shifts, including sleepovers.
• Current driver’s license and access to a reliable vehicle.

Responsibilities

• Deliver person-centred, active support to people with disability.
• Assist with personal care, meals, household tasks and daily routines to promote dignity.
• Support community participation, social connection, appointments and recreational activities.
• Work from clear care plans and behaviour support plans to keep expectations clear.
• Maintain accurate documentation that supports quality and continuity of care.
• Build positive relationships with families, carers and wider support networks.
