Job summary

St Vincent's Health Australia invites applications for a CRESS Aboriginal Health Practitioner in Sydney. This max-term, part-time role delivers culturally safe healthcare to Aboriginal people experiencing homelessness within an assertive outreach model.

You will work with the CRESS team, provide clinical assessments, advocacy, and care coordination to improve housing and health outcomes. Aboriginal descent and relevant qualifications are essential.

Qualifications

  • Must be of Aboriginal and/or Torres Strait Islander descent and able to liaise with local communities.
  • Hold or work towards a Certificate IV in Aboriginal Primary Health Care (Practice) and be eligible for AHPRA registration.
  • Experience with homelessness and social health barriers relevant to Aboriginal communities.
  • Strong verbal and written communication across healthcare and community settings.
  • Ability to work autonomously and within a multidisciplinary team.

Responsibilities

  • Provide culturally safe holistic healthcare to Aboriginal people experiencing homelessness.
  • Collaborate with clients, CRESS Coordinator, and multidisciplinary teams to deliver care and coordination.
  • Apply trauma-informed and harm-reduction approaches in care planning and advocacy.
  • Develop partnerships with Aboriginal communities and service providers to improve access and outcomes.
  • Contribute to service development, quality improvement, research, and governance.
