Job summary

St Vincent’s Hospital Sydney is seeking a Clinical Nurse Specialist (Grade 2) to lead the centralised intake function within Chronic and Complex Care Service in Darlinghurst. You will coordinate referrals, triage and service access with multidisciplinary teams to ensure timely, patient-centred care.

The role requires a NSW RN registration, post-graduate qualifications, and at least three years’ experience, including chronic heart failure or cardiac care.

Qualifications

• Currently registered as a Registered Nurse (Division 1) with AHPRA.
• Post-registration qualifications and at least three years’ experience in the specified post‑graduate area.
• Current unrestricted NSW Driver’s Licence.
• Demonstrated ability to work within the interdisciplinary team at a senior level.
• Ability to provide chronic respiratory input, health coaching and education to clients in multiple clinical settings including within their own homes.
• Previous experience providing assessment and treatment to patients with chronic heart failure or a cardiac background.

Responsibilities

• Lead the centralised intake function, coordinating referrals, clinical triage, prioritisation and allocation of referrals to ensure timely access to community-based services.
• Manage service demand and waitlists by monitoring referral activity, prioritising care needs and reducing waiting times and optimise service capacity.
• Provide expert clinical leadership and decision-making, ensuring referrals are assessed and allocated per clinical need and service criteria.
• Collaborate with multidisciplinary teams, referrers, GPs, hospitals and community providers for coordinated care.
• Monitor, analyse and report on referral, waitlist and service activity data to identify trends and inform workforce planning.
• Practice with autonomy and accountability, demonstrating advanced nursing expertise and delegation.

About the role

Provide advanced clinical leadership to the community services intake function, ensuring timely, equitable and patient‑centred access to care. • Manage service demand and waitlists by monitoring referral activity, prioritising care needs and reduce waiting times and optimise service capacity.


• Provide expert clinical leadership and decision‑making, ensuring referrals are assessed and allocated in accordance with clinical need, service criteria and best practice standards.


• Collaborate with multidisciplinary teams, referrers, general practitioners, hospitals and community providers to facilitate coordinated, seamless and patient‑centred care.


• Monitor, analyse and report on referral, waitlist and service activity data to identify trends, inform workforce planning and support continuous service improvement.


• Practice with a high degree of autonomy and accountability, demonstrating advanced nursing expertise, leadership and delegation in accordance with professional standards and organisational requirements.

About you

• Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency


• Relevant post‑registration qualifications and at least three years’ experience working in the clinical area of their specified post‑graduate qualification


• Current unrestricted NSW Driver’s Licence


• Demonstrated ability to work within the interdisciplinary team at a senior level


• Ability to provide chronic respiratory input, health coaching and education to clients in multiple clinical settings including within their own homes


• Previous experience providing assessment and treatment to patients with chronic heart failure or experience managing patients with a cardiac background
