St Vincent’s Hospital Sydney is seeking a Clinical Nurse Specialist (Grade 2) – Centralised Intake Coordinator for the Chronic and Complex Care Service in Darlinghurst. This max term contract full time position leads the intake function, coordinates referrals and triage, and ensures timely access to community-based services.

The role requires advanced nursing leadership, collaboration with multidisciplinary teams and a track record in chronic disease management.

Qualifications

• Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
• Post-registration qualifications in relevant field
• At least three years’ experience in the clinical area of post‑graduate qualification
• Current unrestricted NSW Driver’s Licence
• Ability to work within an interdisciplinary team at a senior level
• Ability to provide chronic respiratory input, health coaching and education
• Experience with chronic heart failure or cardiac patient management

Responsibilities

• Lead the centralised intake function, coordinating referrals, clinical triage, prioritisation and allocation to ensure timely access to community-based services.
• Manage service demand and waitlists by monitoring referral activity and reducing waiting times.
• Provide expert clinical leadership and decision-making in line with clinical need and best practice standards.
• Collaborate with multidisciplinary teams, referrers, GPs, hospitals and community providers to ensure coordinated care.
• Monitor, analyse and report referral, waitlist and service activity data to inform workforce planning and improvement.

About the role

Provide advanced clinical leadership to the community services intake function, ensuring timely, equitable and patient‑centred access to care. Working collaboratively with multidisciplinary teams and key stakeholders, you will coordinate referrals, clinical triage and service access while supporting quality outcomes and operational efficiency.

• Lead the centralised intake function, coordinating the referral, clinical triage, prioritisation and allocation of referrals to ensure timely access to community‑based services.


• Manage service demand and waitlists by monitoring referral activity, prioritising care needs and reduce waiting times and optimise service capacity.


• Provide expert clinical leadership and decision‑making, ensuring referrals are assessed and allocated in accordance with clinical need, service criteria and best practice standards.


• Collaborate with multidisciplinary teams, referrers, general practitioners, hospitals and community providers to facilitate coordinated, seamless and patient‑centred care.


• Monitor, analyse and report on referral, waitlist and service activity data to identify trends, inform workforce planning and support continuous service improvement.


• Practice with a high degree of autonomy and accountability, demonstrating advanced nursing expertise, leadership and delegation in accordance with professional standards and organisational requirements.

About you

• Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency


• Relevant post‑registration qualifications and at least three years’ experience working in the clinical area of their specified post‑graduate qualification


• Current unrestricted NSW Driver’s Licence


• Demonstrated ability to work within the interdisciplinary team at a senior level


• Ability to provide chronic respiratory input, health coaching and education to clients in multiple clinical settings including within their own homes


• Previous experience providing assessment and treatment to patients with chronic heart failure or experience managing patients with a cardiac background
