Job summary

Darling Downs Health in Inglewood is seeking a Coordinator Home Care to lead community-based services, support a dedicated team and help older people stay safe, independent and connected in their own homes.

You will coordinate person-centred home care, build partnerships with clients, families and health professionals, and lead a team to deliver safe, high-quality services while identifying opportunities to improve delivery and client experience.

Qualifications

• Experience coordinating community, aged care or home-based support services.
• Current C class driver's licence and willingness to travel as required.
• Qualification in individual support, ageing or related field is a plus, especially for rural or regional work.

Responsibilities

• Coordinating person-centred home care services that support client goals, wellbeing and independence.
• Building strong partnerships with clients, families, health professionals and community organisations to deliver seamless care.
