Job summary

Northside Community Service is seeking a Consumer Intake Officer to join our Entry & Temporary Supports team. You will be the first point of contact for clients, guiding referrals through to service delivery across aged and community care programs, and ensuring care plans stay on track as needs change.

You will assess referrals, identify client strengths and risks, develop support plans, and coordinate service delivery with internal and external providers.

Qualifications

• Proven intake/assessment/service coordination experience in aged or community care.
• Understanding of CHSP and Support at Home aged care programs.
• Strong communication and ability to connect with diverse backgrounds.
• Ability to manage a caseload independently and flexibly.
• Competence with client management systems and digital tools.
• Driver's license and access to a reliable vehicle.
• Current First Aid/CPR and WWVP registration; Police Check.

Responsibilities

• Develop and monitor support plans aligned to goals and requirements.
• Coordinate service delivery with internal and external providers and monitor bookings.

Job Description

Northside Community Service is looking for a Consumer Intake Officer to join our Entry & Temporary Supports team. You'll be the first point of contact for people reaching out for support, guiding them from referral through to service delivery across our aged and community care programs, and making sure existing clients' care stays on track as their needs change.

Key Responsibilities

• Receive referrals and conduct timely intake assessments via phone or in person across our aged and community care programs
• Identify client strengths, needs, risks, and eligibility for the appropriate program.
• Develop support plans with clients, aligned to their goals and program requirements.
• Coordinate service delivery through internal and external providers and monitor bookings.
• Regularly review existing clients' care plans and service agreements, updating them as circumstances or needs change.
• Maintain regular contact with clients to confirm satisfaction and address issues early.
• Maintain accurate client records and reporting in line with funding requirements.

What you'll bring

• Proven experience in a similar intake, assessment, or service coordination role within aged or community care.
• Understanding of aged care programs such as CHSP and Support at Home.
• Strong communication skills and the ability to connect with people from diverse backgrounds.
• Confidence managing a caseload independently and flexibly.
• Competence with client management systems and everyday digital tools.
• Certificate III or IV in Aged Care, Disability, or Community Services (or equivalent).
• Tertiary qualifications in allied health, nursing, or a related field would be highly regarded.
• Current driver's licence and access to a reliable, insured vehicle.
• Current First Aid/CPR (or willingness to obtain), National Police Check, and Working with Vulnerable People registration.
