• Person-centered planning: chat with client and family to understand goals and routines.
• Provide complex physical care and assist with mobility, transfers, daily tasks, and devices.
• Support medical plans, monitor changes, and liaise with the care team as needed.
• Facilitate social time and community access aligned with client interests.
• Support autonomy, consent, and independence while respecting client choices.
• Demonstrate cultural humility and respect diverse backgrounds and beliefs.
