Responsibilities

• Person-centered planning with the client and family.
• Provide complex physical care, mobility, transfers and daily tasks.
• Support medical plans and liaise with care team.
• Foster social time and community participation.
• Support autonomy and advocacy while respecting consent.
• Show cultural humility and respect diverse backgrounds.
