Responsibilities

• Person-centered planning: chat with client and family to understand goals and routines.
• Assist with mobility, transfers, positioning, daily tasks, and devices as per plan.
• Monitor client health changes and liaise with care team.
• Facilitate social time and community access aligned with interests.
• Support autonomy, advocate for clients, respect consent, and promote independence.
