Job summary

Claro Disability Services is seeking compassionate care workers to support clients with complex needs across mobility, daily tasks, and medical supports. You will work closely with clients, families and care teams to deliver person-centred plans and meaningful daily routines.

You’ll tailor supports to individual goals, adapt to changing needs, and help clients participate in social activities while respecting cultural backgrounds and personal preferences.

Qualifications

• Role requires a person-centered, warm, patient and caring approach.
• Experience or interest in complex care, disability and health nuances helpful.
• Strong communication, listening, and teamwork skills.

Responsibilities

• Person-centered planning: chat with client and advocates to understand goals and routines.
• Provide complex physical care including mobility, transfers and daily tasks per plan.
• Support medical needs: monitor changes, follow clinical plans, liaise with care team.
• Facilitate social time and community access aligned with client interests.
• Support autonomy and advocacy, respect consent, promote independence.
• Respect cultural backgrounds and values in care delivery.
