Job summary

Claro Disability Services is looking for compassionate care professionals to support clients with complex needs. You will engage with clients and families to set goals, coordinate mobility and daily tasks, and liaise with medical teams as plans evolve.

The role emphasizes autonomy, cultural respect, and meaningful community participation, with a flexible roster across client shifts. Experience in disability or aged care is preferred.

Qualifications

• Minimum 1–3 years’ experience in Disability or Aged Care.
• Ability to work with people with complex care needs and diverse backgrounds.
• Excellent listener and responsive to client goals and family input.

Responsibilities

• Chat with the client and family to understand goals and routines.
• Support mobility, daily tasks, and use of assistive devices as outlined.
• Monitor medical plans and liaise with the care team as things shift.
• Facilitate social participation and community access aligned with interests.
• Encourage autonomy, respect consent, and empower decision-making.
• Demonstrate cultural humility and respect for diverse backgrounds.
• Collaborate with a multidisciplinary team to deliver coordinated supports.
