We deliver individualised supports intended to promote independence, inclusion and positive outcomes for people experiencing disability, disadvantage or other barriers to community participation.

About the position Value Care Limited is seeking a qualified and experienced Community Worker to join its team in Swan Hill, Victoria. The Community Worker will assess clients’ social and practical needs, develop appropriate support strategies, connect clients with community services and coordinate programs that promote independence and participation.

The position involves working with clients, families, community organisations and external service providers. Key responsibilities Conduct client assessments and implement individualised action plans addressing identified social, practical and economic needs.

Identify gaps in available community services and liaise with government agencies, community organisations and service providers to improve clients’ access to appropriate support. Provide information, advocacy and practical support to disadvantaged individuals and groups to strengthen inclusion and community participation. Facilitate skill-development activities designed to improve clients’ independence, confidence and everyday living capabilities.

Provide immediate support and arrange appropriate referrals where clients experience crisis, vulnerability or high-risk circumstances. Plan, coordinate and monitor outreach initiatives, community-support activities and relief programs. Assist clients to access appointments, community services, education, employment, recreational programs and social activities.

Consult with clients, families and relevant professionals to identify changing needs and review the effectiveness of support plans. Monitor clients’ progress and maintain accurate case notes, assessments, reports and other service documentation. Research and analyse social issues and community needs affecting clients and the broader local community.

Evaluate the availability and suitability of health, welfare, housing, employment, training, recreation and other community resources. Develop productive relationships with community organisations, support agencies and stakeholder groups and encourage their cooperation in addressing identified needs. Assist in developing community programs, service procedures, reports, budgets and operational policies.

Promote clients’ rights, dignity, choice, independence, cultural safety and community participation. Comply with organisational policies, privacy obligations, safeguarding requirements and applicable workplace health and safety standards. Qualifications and experience

The successful applicant must have: A relevant Diploma-level qualification or higher in community services, social services, welfare or a closely related discipline. At least two years of relevant experience in a Community Worker or comparable community-services position.

Experience assessing client needs and developing, implementing and reviewing individualised support plans. Knowledge of disability, aged-care, welfare and community-service systems. Experience coordinating referrals and working with community organisations and external service providers.

Strong written communication and recordkeeping skills. The ability to prepare professional case notes, client reports and program documentation. Strong interpersonal skills and the ability to work respectfully with people from diverse cultural and social backgrounds.

The ability to work independently and as part of a multidisciplinary team. A valid Australian driver licence and willingness to travel where required. Current checks and clearances required for the position, including a National Police Check, NDIS Worker Screening Check and

This is a full-time position ordinarily comprising 38 hours per week. The successful applicant may be required to undertake shift work, including work outside ordinary business hours, depending on client and operational needs.
