Job summary

Responsibilities

• Identify gaps in community services and liaise with agencies to improve access to support.
• Provide information, advocacy and practical support to promote inclusion and participation.
• Coordinate outreach initiatives and relief programs.
• Maintain accurate case notes, assessments and program documentation.
• Develop relationships with community organisations and stakeholders.
