We are committed to supporting individuals, families and community groups by connecting them with appropriate welfare, health, housing, employment, training and other community-based services.
The successful candidate will play an important role in assessing client needs, coordinating support services and assisting clients to achieve positive outcomes. Key Duties and Responsibilities may Include but not limited to: Assessing clients' needs and implementing educational, training and support programs. Interviewing clients and assessing the nature and extent of difficulties.
Monitoring and reporting on the progress of clients. Assessing community needs and resources for health, welfare, housing, employment, training and other facilities and services. Liaising with community groups.
Supporting families and providing education and care for children and disabled persons in adult service units. Assisting young people to solve social, emotional and financial problems. Assisting in preparing submissions for funding and resources, and reports to government bodies and other agencies.
Build relationships with local community groups and stakeholders. Respond to crisis situations and provide immediate support. Skills and Experience: At least 1 year of relevant experience in community services, welfare support or a similar role.
Strong understanding of community services, welfare programs and available support resources. Excellent communication, interpersonal and organisational skills. Ability to assess client needs and develop appropriate support plans and programs.
Strong interviewing, case coordination and client support skills. Ability to prepare reports, submissions and client documentation accurately. Ability to liaise effectively with community organisations, government agencies and other service providers.
Ability to work independently and collaboratively within a multidisciplinary team. Strong commitment to confidentiality, professional conduct and client wellbeing. What
