Job summary

cohealth is seeking a Support Work - Homeless & Community Support Worker in Victoria, Australia. You will help vulnerable older people access aged care, housing and community supports to improve wellbeing and independence within a multidisciplinary team.

Applicants should have a relevant tertiary qualification and experience with complex needs, with a focus on human rights and person-centered care. Detailed onboarding and supportive policies are provided.

Qualifications

• Demonstrated experience working with vulnerable people with multiple and complex needs.
• Knowledge of social determinants of health and trauma-informed, strength-based approaches.
• Understanding of case management processes: intake, assessment, care planning, advocacy and referrals.

Responsibilities

• Conduct assertive outreach to identify and engage vulnerable older people who may benefit from the Care Finder program
• Assist clients to register with My Aged Care and access government funded MAC services
• Develop and implement goal-directed support plans reflecting client priorities
• Build trusting relationships with clients using trauma-informed, strengths-based approaches
