ADSSI Limited is recruiting multiple Community Support Workers across Lake Macquarie and the Central Coast to join their Out of Hospital Care services. You will work one-to-one with clients in their homes and community to promote independence and safety.

Roles include personal care, domestic tasks, meal preparation, transport, and social support. A Certificate III in Individual Support and a current NSW driver license are required, with pre-employment checks conducted.

Formación

• Certificate III in Individual Support (Ageing and/or Disability) is required.
• Current NSW Driver Licence is required.
• Registered vehicle with comprehensive insurance and a safe driving record is preferred.
• First Aid and CPR or willingness to obtain prior to commencement.
• National Criminal History Check, WWCC and NDIS Worker Check required.

Responsabilidades

• Personal care and daily living assistance.
• Domestic assistance and household tasks.
• Meal preparation and shopping.
• Transport to appointments.
• Community access and social support.
• Medication assistance, where trained.
• Monitoring client wellbeing and reporting changes.
• Supporting clients following discharge from hospital.
• Helping clients regain confidence and independence at home.
