Job summary

Bara Care Pty Ltd is seeking a dedicated Community Support Worker to join our team on a full-time, permanent basis. This role focuses on supporting individuals with disabilities to develop daily living skills, independence, and active participation in the community.

You will work with clients, families, and other service providers to assess needs, coordinate support, and promote wellbeing. The position offers a competitive hourly salary of $41–$45, depending on experience and qualifications.

Qualifications

• A relevant qualification in Disability Support or related field required.
• Experience in community support or disability care desirable.
• Commitment to supporting people with disabilities and promoting independence.
• Strong communication, interpersonal, and relationship-building skills.
• Proficiency in English; Persian (Farsi) knowledge is an advantage.

Responsibilities

• Assess clients' needs and develop, implement, and review appropriate support, education, and training programs.
• Interview clients to understand their circumstances and assess the nature and extent of their personal, social, and support needs.
• Monitor clients' progress, maintain accurate records, and prepare reports on outcomes and support requirements.
• Refer clients to relevant community organisations, government agencies, and other service providers when additional assistance is required.
• Identify community needs and available resources relating to health, welfare, housing, employment, training, and other support services.
• Liaise with community groups, welfare agencies, government bodies, and private organisations to coordinate services and promote awareness of available resources.
• Support families and assist individuals with disabilities in developing daily living skills, building confidence, and achieving greater independence.
• Provide practical guidance and emotional support to help clients address social, emotional, and other personal challenges.
• Assist in developing and implementing individual support plans in collaboration with clients, their families, and relevant stakeholders.
• Prepare reports, submissions, and supporting documentation to assist with funding applications, resource allocation, and service delivery requirements.
