Job summary

Home Care Assistance Central Coast is seeking a compassionate Care Partner to support older Australians in their own homes across the Central Coast region.

You’ll coordinate services, conduct assessments, and manage care plans to keep clients safe, independent and engaged. The role requires clinical knowledge, budgeting awareness, and strong collaboration with families and health professionals.

Qualifications

• Minimum Certificate IV in Community Services or related field (Aged Care, Case Management, Disability)
• Enrolled Nurses (EN) and Registered Nurses (RN) welcome to apply
• Current national police check or willingness to obtain
• Working rights in Australia
• 3–4 years’ experience in case management of Home Care Packages
• Experience managing a caseload with complex care needs
• Experience budgeting and forecasting client funds/budgets
• Liaising with allied health, GPs, hospitals, My Aged Care and families

Responsibilities

About the Role

As a Care Partner, you will manage a portfolio of Home Care Package clients across the Central Coast region.

You will play a key role in ensuring clients receive the right supports at the right time, while helping them achieve their goals and maintain independence at home.

Your responsibilities will include:

• Conducting client onboarding, assessments and care planning.
• Completing regular care plan reviews and monitoring changing needs.
• Coordinating services and supports in partnership with clients and families.

• Working collaboratively with allied health providers, GPs, hospitals and community services.
• Monitoring client budgets and supporting informed decision‑making.
• Maintaining accurate documentation and compliance with aged care requirements.
• Supporting positive client outcomes through proactive case management and relationship building.

Essential Qualifications

• MinimumCertificate IV in Community Services(or a related field such as Aged Care, Case Management, or Disability)
• Enrolled Nurses (EN) and Registered Nurses (RN) are welcome and encouraged to apply
• Current national police check (or willingness to obtain)

Experience

• Minimum3–4 years' experience in case managementof Home Care Packages, including experience transitioning clients into theSupport at Home (SAH)program
• Demonstrated experience managing a caseload spanningall levels of care, including complex care needs
• Proven experiencemanaging and monitoring client funds/budgets, including forecasting, utilisation tracking, and ensuring services remain within approved budgets
• Experience liaising with a broad range of stakeholders across the aged care ecosystem (e.g. allied health, GPs, hospitals, My Aged Care, service providers, families/representatives)

Knowledge & Technical Skills

• Sound understanding of, and demonstrated ability to apply, theSupport at Home (SAH) programguidelines and requirements
• Strong understanding of Transitioning Home Care Package guidelines, Aged Care Quality Standards, and relevant consumer directed care principles
• Confident and accurate inbudgeting and financial management of client funds
• Good working knowledge of care management systems/software, with a strong overall "sense of systems" (data accuracy, workflows, documentation, compliance)
• Ability to interpret assessed client needs and translate these into an appropriate, funded care plan
