Job summary

Northern Coalfields Community Care Association in Cessnock, NSW is seeking a dedicated Community Care Manager to lead our Support at Home services on a 12-month fixed-term contract.

You will coordinate person-centred care for older clients, work with families and providers, and ensure high standards, compliance, and on-call readiness.

The role requires a Diploma in Community Services or related field and a commitment to continuous improvement.

Qualifications

• Experience in care coordination, case management or a similar role.
• A Diploma of Community Services, Nursing, Aged Care or related qualification.
• Knowledge of CHSP and Support at Home programs.
• Strong communication, organisational and administrative skills.
• Commitment to person-centred care and continuous improvement.
• Appointment is conditional upon a satisfactory criminal record check and influenza vaccination.
