Hiring4Health is seeking an experienced Clinical Case Manager in Inner West Sydney to oversee home-based care with a person-centred approach. In this full-time, hybrid role you will assess needs, plan care, and support clients to maintain independence and wellbeing.

You'll lead a multidisciplinary team, conduct nursing assessments, ensure regulatory compliance, and build trusting relationships with clients and families.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Experience in community or aged care settings.
• Current driver's license and a reliable, insured vehicle.
• Full Australian working rights.
• National Police Check and NDIS Worker Screening Check (or ability to obtain).
