Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to provide in-home care for clients in the community, supporting independence and improving quality of life. You’ll spend your days on the road visiting clients in their homes and carrying out activities per their Care Plans.

Hours are mainly Monday to Friday 9am–5pm. Must have CPR/First Aid, a current driver’s license and access to a vehicle, and be willing to drive between clients.

Qualifications

• Experience in Disability, Aged and Community sector is ideal but not essential.
• Current CPR and First Aid Certificate (required before you start)
• Open Driver's License and access to vehicle
• Car Insurance that is in your name

Responsibilities

• In home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing
• Must be available to drive between clients with your own vehicle
• Providing care in accordance with individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments
