Job summary

UnitingCare Queensland is hiring a Supported Living Facilitator to deliver in-home care for clients in the Plainland area. You will drive between visits, support independence, and follow individualized Care Plans to enhance daily living activities.

Role requires CPR/First Aid, NDIS screening, and a current driving licence. Casual term with on-going travel across Plainland and surrounding communities. Be part of a supportive FaDS team making a real difference.

Qualifications

• In home personal care including showering, hygiene, meal prep, house cleaning, laundry and ironing.
• Must be available to drive between clients with own vehicle.
• Hours predominantly Monday to Friday, 9am–5pm.
• Care provided per individual Care Plan.
• Support with community activities, shopping and appointments.

Responsibilities

• Provide in-home personal care and daily living assistance.
• Drive between clients using own vehicle.
• Follow each client's Care Plan and coordinate with the team.
• Support clients with community-based activities and appointments.
