Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to provide on-the-go in-home care and daily living assistance. You will drive between clients, support independence, and help with activities that improve quality of life. Must have a positive attitude and willingness to learn.

Role includes driving to clients, planning activities, and working within Care Plans in a team-focused environment. Emphasis on safety, inclusivity, and client-centred care across Queensland communities.

Qualifications

• NDIS Worker screening is current.
• CPR and First Aid certificates required before you start.
• Open drivers license and access to a vehicle.
• Experience in the Disability, Aged and Community sector is beneficial, but not essential.

Responsibilities

• Provide in-home personal care and daily living assistance (showering, hygiene, meal prep, housekeeping, laundry).
• Spend time with clients on the road visiting them in their homes.
• Follow individualised Care Plans and support community activities.
• Assist with appointments, shopping and transportation.
• Collaborate with team to deliver high-quality support.
