Job summary

UnitingCare Queensland is seeking a casual Supported Living Facilitator to enable clients to live independently at home. You’ll provide in-home care, accompany clients on activities and support with daily living tasks, while driving between homes with your own vehicle.

You’ll work on a schedule Mon–Fri daytime, travel across Plainland and surrounding towns within a coordinated care plan, and contribute to a compassionate, community-focused team committed to safer, happier lives for clients.

Qualifications

• CPR and First Aid certificates are required before starting.
• Open Driver's Licence and access to a vehicle are required.
• Experience in Disability/Community services is beneficial but not essential.
• Ability to communicate clearly in spoken and written English.
• Willingness to travel between Plainland and surrounding towns.

Responsibilities

• In-home personal care and daily living support as per Care Plan.
• Drive between clients using your own vehicle.
• Provide care aligned with individualised Care Plans.
• Support community-based recreational and social activities.

Remuneration: $ 45.28 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team

You’ll provide care to your clients living in the community, supporting their independence and improving their quality of life. With no two days ever the same, you’ll spend your time on the road completing your scheduled visits, interacting one-on-one with clients and their loved ones within their homes completing activities that include:

• In home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing
• Must be available to drive between clients with your own vehicle
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments

What makes a difference for us?

• Available Monday - Friday between 8:30am - 3:00pm
• Willingness to travel between Plainland, Moore, Calvert, Kensington Grove, Fernvale and Laidley
• Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers
• Able to effectively communicate in spoken and written English
• Experience working in the Disability, Aged and Community sector is ideal but not essential – as long as you have positive attitude and a willingness to learn
• Current CPR and First Aid Certificate (required before you start)
• Open Drivers License and access to vehicle
