Job summary

UnitingCare Queensland is seeking a Supported Living Facilitator to provide community-based care for clients in Plainland and surrounding areas. You will assist with daily living activities, mobility, and social participation, following personalised Care Plans.

Role involves driving between clients, using your own vehicle, and working mainly Monday to Friday, 9am–5pm. CPR/First Aid certification is required prior to start; a valid licence and car insurance in your name are preferred.

Qualifications

• CPR and First Aid certificates are required before start.
• Open Driver's License with access to a vehicle.
• Car Insurance in your name is preferred.
• Able to communicate effectively in spoken and written English.
• Experience in Disability/Aged/Community sector is helpful but not essential.

Responsibilities

• Provide in-home personal care and daily living assistance.
• Travel between clients using your own vehicle.
• Work mostly Monday to Friday, 9am–5pm.
• Follow individual Care Plans and support independence.

• Remuneration: $ 45.28 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team

You’ll provide care to your clients living in the community, supporting their independence and improving your quality of life. With no two days ever the same, you’ll spend your time on the road completing your scheduled visits, interacting one-on-one with clients and their loved ones within their homes completing activities that include:

• In home personal care, daily living assistance including showering, personal hygiene, meal preparation, house cleaning, laundry & ironing
• Must be available to drive between clients with your own vehicle
• Hours are mainly Monday to Friday between the hours of 9am-5pm
• Providing care in accordance with their individualised Care Plan
• Supporting with community based recreational and social activities
• Transportation and assistance with shopping and appointments

What makes a difference for us?

• Available Monday - Friday between 8:30am - 3:00pm
• Wilingness to travel between Plainland, Moore, Calvert, Kensignton Grove, Fernvale and Laidley
• Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers
• Able to effectively communicate in spoken and written English
• Experience working in the Disability, Aged and Community sector is ideal but not essential - as long as you have positive attitude and a willingness to learn
• Current CPR and First Aid Certificate (required before you start)
• Open Drivers License and access to vehicle
• Car Insurance that is in your name
