Job summary

Peregrine Partners on the Gold Coast is seeking an experienced Clinical Case Manager and Registered Nurse to provide clinical leadership, assessments and oversight for participants receiving health and disability supports.

You will collaborate with Specialist Care Coordinators, conduct comprehensive clinical assessments, monitor changes, prepare reports and ensure safe care delivery in line with Delegation of Care requirements.

Qualifications

• Bachelor of Nursing or equivalent is required.
• Current AHPRA registration as a Registered Nurse.
• Current First Aid and CPR.
• Current Criminal History Check and Working with Children Check.
• Completion of the NDIS Worker Orientation Module.
• Valid Australian driver’s licence.
• Minimum two years' experience in a similar role.
• Experience working with people with disability, including children and young adults.
• Strong communication, organisational and clinical documentation skills.

Responsibilities

• Conduct comprehensive clinical assessments and ongoing reviews.
• Monitor changes in participants' clinical presentation and care needs.
• Provide clinical recommendations regarding supports, staffing, skill mix and escalation requirements.
• Develop, review and clinically authorise Delegation of Care documentation.
• Provide clinical education and guidance to RNs, ENs and Disability Support Workers.
• Prepare clinical reports, summaries and risk statements.
• Provide clinical input into NDIS-related processes.
• Support community nursing assessments including wound care, catheter care and continence.
• Identify and escalate clinical risks through governance pathways.
• Collaborate with internal teams and Specialist Care Coordinators.

Clinical Case Manager - Registered Nurse
Our client is a purpose-led health and disability organisation delivering specialist mental health, nursing and NDIS services. They are seeking an experienced Clinical Case Manager to provide clinical leadership, assessment and oversight for participants receiving health and disability supports.
Working closely with Specialist Care Coordinators, the Clinical Case Manager will ensure participant care is clinically appropriate, safe and aligned with Delegation of Care requirements. The role is responsible for the clinical assessment, monitoring, reporting and decision-making that underpins safe service delivery.
Key responsibilities include:

• Conducting comprehensive clinical assessments and ongoing reviews.
• Monitoring changes in participants' clinical presentation and care needs.
