Job summary

Evolve Talent is seeking a Clinical Care Coordinator to lead clinical care for residents in a coastal Mid North Coast NSW facility. You will work with a leadership team to mentor nursing and care staff, overseeing governance and AN-ACC funding while ensuring clear communication across the multidisciplinary team.

Your RN leadership experience and knowledge of accreditation standards will support ongoing staff development and high-quality resident care in a supportive environment.

Qualifications

• AHPRA Registered Nurse registration and leadership experience.
• Knowledge of accreditation standards and quality indicators.
• Ability to identify and improve areas of clinical risk and quality guidelines.

Responsibilities

• Provide clinical guidance to residents and care staff.
• Undertake resident assessments and case conferencing.
• Accurate documentation and ongoing review of records.
• Motivate and develop clinical team members.

About the Role:

As Clinical Care Coordinator you will be primarily responsible for the clinical care of care recipients within the facility. Working within the leadership team, supported by an experienced and passionate residential & care manager within this medium sized care home within the coastal Mid North coast!

You will provide clinical leadership to nursing and care staff, mentoring and inspiring staff to provide outstanding care to all residents whilst supporting with their ongoing development and career progression

You will utilise your previous experience to oversee the clinical governance of the facility and optimise AN-ACC funding. You will coordinate your team and ensure safe, clear and effective communication across the multidisciplinary team.
