Job summary

Sidekicker is seeking a Clinical Care Partner to provide holistic care management for Home Care Packages in Brisbane. You will manage a caseload of 40–50 clients with a consumer-directed model, coordinating plans with families and carers to support independence.

Hybrid role based in Fortitude Valley: in-office for the first month, then two days a week from home. You will work under the Home Care Nurse Lead, ensuring care plans meet quality standards and budgets.

Qualifications

• Current and active AHPRA registration required; RN/OT/PT preferred.
• Experience in case management and coordinating individualised packaged care (HCP).
• Able to autonomously manage a caseload of 40–50 clients.
• Strong interpersonal and collaborative problem-solving skills.

Responsibilities

• Develop, implement, and annually review person-centred care plans with clients, families and carers.
• Ensure packages operate within budgets and funding requirements with regular reporting.
• Provide required clinical services under supervision of the Home Care Nurse Lead.
• Collaborate with Rostering Team to identify required skills for Support Workers.
• Lead case conferences, family and interagency meetings for coordinated care.
• Maintain accurate client records in the management system and comply with Aged Care Standards.

Join the team as a Clinical Care Partner. In this role, you will provide holistic care management and administrative coordination for clients receiving individualised Home Care Packages (HCP). Managing a typical caseload of 40-50 clients, you will implement a person-centred, consumer-directed model that empowers clients to achieve their goals and live independently in their own homes.

Role: Clinical Care Partner (Care Manager)

Location: Fortitude Valley, Brisbane (Hybrid: In-office full-time for the first month, then 2 days WFH)

Pay Rate: $66.79 per hour

Employment Type: Ongoing casual with opportunity for perm work

Key Responsibilities

• Care Planning & Coordination: Develop, implement, and annually review person-centred care plans collaboratively with clients, their families, and carers.
• Budget Management: Ensure all individual packages operate within approved budgets and funding requirements, completing regular financial reporting.
• Clinical Support: Provide a minimal number of standard clinical/nursing services applicable to your specific qualifications, under the supervision of the Home Care Nurse Lead.
• Team Guidance: Collaborate with Program Coordinators and the Rostering Team to identify the skills Support Workers need to safely and effectively help clients achieve their goals.
• Stakeholder Engagement: Organise and lead case conferences, family meetings, and interagency meetings to ensure collaborative, culturally appropriate care.
• Compliance & Administration: Maintain accurate, up-to-date records in the client management system while ensuring all practices comply with Aged Care Quality Standards and HCP Guidelines.

What We Are Looking For

• Clinical Qualifications: A current and active AHPRA registration is required. Our strong preference is for a Registered Nurse (RN), Occupational Therapist (OT), or Physiotherapist.
• Case Management Experience: Demonstrated experience in case management and coordinating individualised packaged care (HCP) in a community or aged care environment.
• Caseload Capacity: Highly organised and comfortable autonomously managing an ongoing caseload of 40-50 clients.
• Communication: Exceptional interpersonal skills with a focus on collaborative problem-solving, restorative practices, and promoting client independence.
