Based in Wangaratta, this full-time Clinical Care Partner role is an opportunity to use your clinical expertise in a meaningful, person-centred way. As a registered health professional, you’ll provide safe, clinically informed care to participants with identified clinical needs, completing comprehensive assessments, developing individualised treatment and reablement plans, and delivering targeted clinical nursing services that promote wellbeing, restorative care and functional independence. Working within your scope of practice, AHPRA registration and Support at Home requirements, you’ll build meaningful relationships with participants and their families while delivering care that reflects their individual needs, choices and goals.
This is more than a traditional clinical role — it’s an opportunity to provide clinical oversight, intervention, advice and leadership. You’ll work closely with Care Partners, the broader clinical team, GPs, allied health professionals and external health providers to deliver coordinated, evidence-based care. You’ll manage a portfolio of participants requiring clinical support and play a key role in Restorative Care Pathways, providing short-term clinical responsibility and supporting a smooth transition back to the Care Partner for ongoing case management.
