Healthii People is seeking a Clinical Care Partner to support Footprints Community’s ageing services in the Brisbane region. You will manage care coordination for Home Care Package clients, provide administrative support, and work within a person-centred consumer directed model to help clients live independently.

Ideal candidates hold tertiary qualifications in Nursing or allied health, with current AHPRA registration and strong knowledge of the Aged Care Quality Standards, including Support at

• Tertiary qualifications in Nursing, Allied Health or a related clinical field.
• Current AHPRA registration
• Strong knowledge of the Aged Care Quality Standards, including Support at Home
• A person-centred clinical leader passionate about helping older people live well

Responsabilités

• Provide effective and efficient care management and coordination of services for Home Care Package clients.
• Deliver administrative support to enable smooth service delivery.
• Support clients to achieve goals and live independently at home through a person-centred approach.

As the Clinical Care Partner, you will provide effective and efficient care management, administrative support, and coordination of services for clients receiving individualised Home Care Packages. Through provision of holistic care management for individually funded clients, the Clinical Care Partner will implement a person-centred consumer directed model to support clients in achieving their goals and enable them to live independently in their own homes.
