Job summary

• Current National Police Certificate.

Responsibilities

• Serve as the client’s primary contact for delivery of services and liaison with carers and family.
• Conduct home visits to assess client needs, goals, and monitor progress of care plans.
• Facilitate and monitor individualised care plans and budgets within guidelines.
• Build collaborative relationships with clients, families and care partners.
