The overall purpose of the Practitioner - Out of Home Care is to provide case management to children or young people within residential care services or programs. As a Practitioner - Out of Home Care, you will provide case management and holistic support to children and young people in residential care, helping them achieve positive outcomes through education, life skills development, emotional wellbeing, family connections and community participation. Working collaboratively with families, carers and key stakeholders, you will play a vital role in creating safe, stable and nurturing environments where young people can thrive.
This is a full-time opportunity based in Hervey Bay working 38 hours per week.
Pay rate of $53.42 per hour plus additional employee benefits.
