Job summary

Direct Care Resources is seeking a Case Manager (Manager for Care Planning and Practice) to lead a dedicated team delivering high-quality care across Disability and Child Safety clients in the Ipswich region of Queensland.

You will oversee care planning, ensure regulatory compliance, and drive a positive team culture, coordinating with health professionals and senior managers to implement effective care practices.

Qualifications

• Bachelor's degree mandatory in community services or related field.
• Minimum 2 years' experience in care/case management.
• Knowledge of Disability Care, Forensic Disability, Child Protection, Aged Care, or Health Care legislation.
• Queensland Driver Licence required.
• First Aid & CPR, Blue Card, NDIS Worker Screening Check, and LCS2 Background Check held or willing to obtain.

Responsibilities

• Provide high-quality care management advice to principals and executive management.
• Oversee care management for adults with intellectual disabilities and child safety cases.
• Develop and implement care planning and practice frameworks.
• Produce clear reporting within required regulatory timeframes.
• Ensure accurate behaviour recording and staff knowledge of client behaviours.
• Coordinate with health professionals to create treatment regimens.
• Conduct supervision, training, and monthly client needs assessments.
• Participate in after-hours advice roster for critical incidents.
