Job summary

Breakthru, a disability, mental health, and employment services provider in Port Macquarie, NSW, seeks a casual team member for 25–35 hours per week with open availability. You will work in the community with clients in homes, workplaces, and local neighbourhoods to support daily living, education, employment, and social participation.

Qualifications

• Criminal record check (essential or willing to obtain)
• Working with children’s check (essential or willing to obtain)
• NDIS Workers screening clearance (essential or willing to obtain)

Responsibilities

• Accessing the community for shopping, appointments and day-to-day activities
• Participation in a range of scheduled activities such as cooking, art, outdoor physical activities and excursions
• Engagement in social events
• Transportation assistance where required
• Meal preparation and other simple in-home supports
• Responsible for all aspects of personal care
• Providing goal focused supports for activities, Programs, community supports
• In home supports
• Transport and travel
• Supports related to specific health needs
• Report writing including daily case notes
• Your role will go beyond daily care - promoting independence, dignity, and choice in a safe, structured, therapeutic environment
