Job summary

Breakthru is seeking a casual Community Disability Support Worker to join our Port Macquarie team, delivering person-centred supports in homes, workplaces, and community settings. You will help clients access daily activities, education, employment, and social events, while building real connections and supporting goals.

The role emphasizes independence, dignity, and choice, with flexibility to align with individuals’ needs and a supportive, values-driven workplace culture.

Qualifications

• Criminal record check required or willing to obtain.
• Working with children’s check required or willing to obtain.
• NDIS Workers screening clearance required or willing to obtain.

Responsibilities

• Accessing the community for shopping, appointments and day-to-day activities.
• Participation in a range of scheduled activities such as cooking, art, outdoor physical activities and excursions.
• Engagement in social events.
• Transportation assistance where required.
• Meal preparation and other simple in-home supports.
• Responsible for all aspects of personal care.
• Providing goal focused supports for activities, Programs, community supports.
• In home supports.
• Transport and travel.
• Supports related to specific health needs.
• Report writing including daily case notes.
• Role will promote independence, dignity, and choice while maintaining a safe, structured, and therapeutic environment.

About the role…
