Job summary

Breakthru in Port Macquarie is seeking a casual team member to join our local team for 25-35 hours per week with open availability. You will support individuals in their homes, workplaces, and communities, helping them achieve goals in real-world settings.

The role focuses on personal care, community access, activities, transport, meal prep, and reporting, with an emphasis on independence, dignity, and a safe, structured environment.

Qualifications

• Criminal record check required or willing to obtain
• Working with children’s check required or willing to obtain
• NDIS Workers screening clearance required or willing to obtain

Responsibilities

• Accessing the community for shopping, appointments and day-to-day activities
• Participation in a range of scheduled activities such as cooking, art, outdoor physical activities and excursions
• Engagement in social events
• Transportation assistance where required
• Meal preparation and other simple in-home supports
• Responsible for all aspects of personal care
• Providing goal focused supports for activities, Programs, community supports
• In home supports
• Transport and travel
• Supports related to specific health needs
• Report writing including daily case notes
• Role promotes independence, dignity, and choice in a safe, structured, therapeutic environment
