Kyabra Community Association is seeking a Case Worker to provide case management and coordinate disability and housing support services for community members with complex needs. You will work within an integrated team to connect individuals to supports and navigate systems that impact their lives.

The role requires a tertiary qualification and experience in community services, with responsibilities across intake, assessment, and service coordination.

Qualifications

• Tertiary qualification (bachelor's degree or higher) in Human Services, Social Work, and/or another relevant field.

Responsibilities

• The Case Worker will ensure that the most appropriate and effective support services are provided to a range of individuals and families through case work that supports their aspirations and promotes meaningful change in their lives.
