Job summary

Kyabra Community Association invites applications for a fixed-term full-time Case Worker to join the Case Management and Coordination team in Queensland. The role delivers disability support services (NDIS) and housing support to a diverse caseload, using a strengths-based approach to help clients achieve personal goals.

The successful candidate will hold a tertiary degree in Human Services or Social Work, possess strong communication and organisational skills, and be able to work autonomously

Qualifications

• Tertiary qualification in Human Services, Social Work or related field.
• Demonstrated experience in community services or similar settings.
• Understanding of relevant human services legislation and rights-based approaches.

Responsibilities

• Provide case management support and coordinate services for individuals and families.
• Deliver disability and housing support services as part of an integrated care model.
• Advise on eligibility, intake, assessment and referrals to suitable services.
• Maintain compliance with relevant legislation and organisational policies.
