Job summary

Kyabra Community Association in Brisbane's south-east (Runcorn) is seeking a Case Worker to deliver integrated case management and coordinate supports for people facing complex circumstances. You will help clients identify goals, navigate housing and disability services, and connect them with appropriate community resources.

The role requires a tertiary qualification in human services or social work, strong communication, and the ability to work independently or with teams in a strengths-based

Qualifications

• Tertiary qualification (bachelor's degree or higher) in Human Services, Social Work, and/or another relevant field.
• Experience in the community service sector in intake and assessment, case management, disability support, housing support services, child protection, etc.
• Understanding of the principles of social justice and ability to work in ways that focus on people’s strengths.
• Strong communication, interpersonal and organisational skills, including the ability to work autonomously or in a team.

Responsibilities

• Provide case management support to and coordinate services for individuals and families with complex needs.
• Ensure relevant legislation is understood and applied (e.g., Disability Services and Inclusion Act 2023, tenancy acts).
• Develop and maintain client-centred supports and advocate for housing and disability outcomes.
• Collaborate with internal teams to ensure appropriate access to services.
