Job summary

Kyabra Community Association Inc. is seeking a Case Worker to deliver case management and coordinate disability and housing services for a diverse community.

You will work within an integrated team to support individuals and families, helping them access essential supports and achieve personal goals. The role requires a tertiary qualification in Human Services or Social Work, strong communication skills and the ability to work autonomously or as part of a collaborative team.

Qualifications

• Bachelor's degree or higher in Human Services, Social Work, or another relevant field.
• Experience in community service sectors including intake, assessment, case management, disability and housing support.
• Knowledge of Disability Services and Inclusion Act (2023) and Residential Tenancies laws; commitment to social justice.

Responsibilities

• Provide case management and coordinate services for individuals and families.
• Coordinate disability support and housing services for NDIS participants and other community members.
