Job summary

Kyabra Community Association in Queensland seeks a Case Worker to support people with complex needs through Disability Support via NDIS coordination and Housing Support services. The role sits in the Case Management and Coordination team and uses a strengths-based approach to help individuals navigate services and achieve personal goals.

The position is a full-time, fixed-term role (until 30 June 2027) based in Runcorn QLD with flexible options for part-time applicants.

Qualifications

• Tertiary qualification (bachelor's degree or higher) in Human Services, Social Work, and/or another relevant field.
• Experience in the community service sector including intake and assessment, case management, disability support, housing support services, or child protection.
• Knowledge of Disability Services and Inclusion Act (2023) and Residential Tenancies and Rooming Accommodation Act (2008) and related human services legislation.

Responsibilities

• The Case Worker will ensure the most appropriate and effective support services are provided to a range of individuals and families through case work.
• Provide Disability Support Services via Support Coordination to NDIS participants and Housing Support Services to a caseload of community members.
• Work within the Case Management and Coordination team to support clients and communities.
