Job summary

SilverChain in Adelaide is seeking a skilled Registered Nurse to step into a Case Coordinator role within our community team. You’ll manage your own caseload, provide clinical leadership, and mentor staff, while working with the Clinical Nurse Manager to ensure safe, high-quality home care.

This full-time role offers career progression, a strong hands-on clinical element, and a supportive environment focused on outcomes. Based at Parafield, shifts Monday–Friday with no weekend work.

Qualifications

  • Bachelor of Nursing with current AHPRA registration.
  • Minimum five years post-graduate nursing experience with clinical leadership.
  • Proven ability to manage resources, meet KPIs and support quality outcomes.
  • Mentoring, coaching and staff development capabilities.
  • Excellent communication, negotiation and problem solving skills.
  • Autonomous work style with multidisciplinary collaboration.
  • A current Australian driver’s licence.

Responsibilities

  • Manage and own a clinical caseload for clients with complex care needs.
  • Provide clinical leadership and mentor staff.
  • Act as a clinical escalation point for deterioration or complex situations.
  • Deliver hands-on clinical care and resolve issues with confidence.
  • Support staff performance, wellbeing and professional development.
  • Conduct monthly audits to ensure quality and continuous improvement.
  • Coordinate resources to deliver person-centred, evidence-based care.
  • Collaborate with the Clinical Nurse Manager to meet service goals.
  • Drive clear documentation and stakeholder engagement.

We are seeking a passionate Case Coordinator to provide clinical coordination and leadership within our community team, supporting clients with complex acute and aged care needs. In this pivotal role, you will manage your own caseload, support and mentor staff, and act as a key clinical escalation point while working closely with the Clinical Nurse Manager to deliver safe, high-quality care.

What we’re looking for

You lead with purpose and professionalism and are motivated to support both clients and colleagues. You will bring:

  • Bachelor of Nursing with current AHPRA registration.
