A leading home care provider in South Australia is seeking a skilled Case Coordinator for Palliative Care. This permanent fulltime position involves leading a dedicated team of nurses, coordinating care resources, and managing clinical needs for community clients. The ideal candidate will have at least 2 years of palliative care experience and a current AHPRA registration. Qualifications

Responsibilities
  • Lead and support a dedicated team of Registered and Enrolled Nurses in a community setting, fostering growth, competence, and emotional wellbeing.
  • Coordinate resources to ensure quality person-centred care.
  • Oversee clinical and other care needs for community Palliative Care clients, including complex clinical issues.
  • Work closely with other teams to ensure effective delivery of palliative care services.
  • Act as an escalation point for clinical challenges.
  • Drive communication through team meetings, stakeholder engagement, and documentation excellence.
  • Support onboarding, mentoring, and student placements to build a strong, future-ready workforce.
What we’re looking for (Qualifications / Experience)
  • Current AHPRA registration, with a minimum 2 years palliative care experience.
  • Demonstrated clinical leadership and resource coordination.
  • Ability to mentor and support staff.
  • Excellent communication, negotiation, and problem-solving skills.
