My Guardian is expanding its clinical leadership team and seeks motivated Registered and Enrolled Nurses to join as Clinical Care Coordinators in Sydney. You will lead care planning, supervise staff, and deliver hands-on clinical care while coordinating with families, hospitals and allied health professionals.

Responsibilities

• Lead Care Planning: conduct home assessments and design individualised care plans.
• Mentor & supervise: provide nursing advice and leadership to the care team.
• Hands-on clinical care: manage wound care, medication administration, and specialised treatments.
• Coordinate continuity: act as the link between clients, families, hospitals and allied health professionals.
• Advocate: support NDIS and My Aged Care compliance to support client goals.
