Job summary

Mission Australia in Nowra is seeking a proactive Carer Support Liaison Officer to join the Carer Gateway team. You will coordinate planned and emergency supports, liaise with Carer Support Planners and external providers, and ensure compliance documentation is up to date.

You will manage provider records, support in-home and facility-based care, and help uplift carer wellbeing through accurate brokerage and timely referrals, while maintaining privacy guidelines.

Qualifications

• Qualifications in Community Services/Health/Welfare/Social Services or 3+ years industry experience.
• Strong knowledge of community care sector and client needs.
• Experience coordinating services and managing providers in health or aged/disability care.
• Excellent attention to detail and compliance documentation practices.
• Experience maintaining databases and records with high accuracy.

Responsibilities

• Coordinate and liaise with Carer Support Planners and service providers for in-home or facility-based supports.
• Coordinate care for carers including planned respite, personal care, and domestic support.
• Provide referral information to service providers per consent and privacy guidelines.
• Network with external stakeholders to maintain positive relationships.
• Source brokered supports and maintain provider records and compliance documentation.
• Ensure providers meet compliance before services commence and maintain client records.
